Patients Bill of Rights
Access to Treatment
Every individual, including minors,
and persons with disabilities, shall be given impartial access to
treatment and appropriate medical care based on individual needs,
without considering his/her race, religion, gender, national origin, or
social class.
Respect
You have the right to receive
respectful treatment from all employees of the MajTerrence Dialysis
Centre, at all times and under all circumstances.
Privacy and
Confidentiality
You have the right to privacy.
The hospital, your doctor, nurse, technician and all members of the
MajTerrence facility will protect your privacy as much as possible.
You have the right to expect treatment records are confidential unless
you have given permission to release information or reporting is
required by law. When the MajTerrence Dialysis Centre releases
records to others, such as insurers, it is emphasized that the records
are confidential.
Personal
Safety/Security
You have a right to receive medical
services in a safe and secure environment. Patients receiving care
at the MajTerrence Dialysis will receive treatment under strict safety
and security guidelines to protect both the patients and the caregivers.
Freedom from Abuse
You have the right to be free from all
forms of abuse or harrassment.
Identity
You have the right to know the
identity and the professional position of any individual who is
providing you with services, as well as the right to know who is in
charge of your treatment. Included here is your right to know
whether there is some professional relationship among the individuals
treating you, as well as the relationship to other health or educational
institutions involved in your treatment.
Information
You have the right to receive from the
professional responsible for the coordination of your care the complete
and current information on your diagnosis, treatment alternatives, risks
and prognosis. That information shall be communicated to you in
such a way that you can be expected to understand. When it is not
considered medically advisable to give this information to you, that
information will be made available to an authorized individual.
Consent
You have the right to be informed
about and to participate in the decisions related to your health and
treatment. Whenever possible, this should be based on a clear and
concise explanation of your condition and of all the implicit technical
procedures, including the possibilities of any risk or serious reactions
and of problems related to your overall medical care.
You will not be subjected to any
procedure without your voluntary, informed consent, or that of your
legally authorized representative. When there are medical
alternatives for your treatment you shall be informed at all times.
Your participation in clinical
training programs shall be by informed consent.
Refusal of Treatment
You can refuse treatment. When
the refusal of treatment by you or your legally authorized
representative interferes with the provision of adequate treatment
according to professional standards, the relationship with you may be
terminated with reasonable prior advance notice.
Respect for Culture
and Religion
You have the right to manifestation of
your cultural and/or religious expressions while being treated, as long
as they do not interfere with the normal activities of the health
facility and do not interfere with other patients interest.
Complaints
You have the right to file a complaint
when you consider your right infringed. You also have the right to
have this complaint investigated and the results communicated to you
within a reasonable amount of time.
Patient's Records
You or your authorized representatives
have the right to access a copy of your records.
